You can tick the right hand side of your repeat prescription request form and drop it in one of the collection boxes provided. If you include a stamped addressed envelope, the signed prescription can be posted back to you after processing. Please allow additional time in this case as the postal service can sometimes be very unreliable. If you request items which have not been authorised by your doctor this will lead to a delay in the processing of the prescription. You can email a prescription request to the surgery on wakccg.middlestowndispensary@nhs.net Please allow 2 full working days for the prescription to be processed once it has reached the surgery.

Any amendments may take longer. Patients are now able to order repeat medication via SystmOnline. To do this you have to call in at one of the surgeries with proof of who you are. Your application will be processed and your personal login details  and password will be sent to you by your preferred method. Please keep these safe. Make sure you bring in 2 forms of identification for this. This allows you access to enable you to order medication 24/7 but still we request 2 working days notice before collection.

If you use a pharmacy to collect and order your repeat medication you can sign up for ETP (Electronic Prescription Service) speak to your authorised pharmacy about this.
